Medley Kentucky Straight Bourbon Whiskey?

First time here so please excuse me if I get the ettiquette wrong. I searched for this on the boards but can't find it anywhere so hope I've not missed a post somewhere.
I've bought this as a present for a good friend who is a bit of a bourbon enthusiast. The shop I bought it from here in England said it came from a private cellar sale and that they have no idea exactly when it is from but the estimate is that it's 40 years old (the bottle, not the whiskey, I appreciate that).
Can any of you help? I'd be delighted if someone could shed some light on this for me.

Even a search for images of this bottle has turned up nothing.
